What is a Digi-Physical Service?

With Digi-physical, your first contact with a doctor is always through a ‘Digital-first’ remote appointment. If your doctor can safely treat you via video or telephone, they will do so. If, however, following your consultation you need (or want!) to see us in person, our doctor can book you in to our ‘in-person services’ within our catchment area. Our in-person services are provided via a combination of scheduled home visits to your door, and clinics.

Dr Shiraz Deen

Originally from Sri Lanka, I trained as a doctor in Manchester and moved to the Oxford Deanary to train as a GP. Since been working as a GP in the Thames Valley area for the past 10 years and can speak English, Sinhala, Tamil, Malay (somewhat). I’m currently working as a NHS GP in Windsor, and a GP Trainer for the Oxford Deanary.

I have long been interested in developing more accessible services, and am the clinical lead of the local extended access service through which we deliver evening and weekend NHS GP appointments out of Windsor and Maidenhead. I co-founded Prime Medicare Ltd because I believe accessible, responsive, patient centred healthcare is essential as we move to a more connected future.

Dr Sufian Jabbar

Dr Jabbar qualified in 2002 from Imperial College school of medicine. He has been a GP at Langley Health Centre since 2009 and at Lee House Surgery in Windsor since 2013. His clinical interest include diabetes, teaching, and commissioning. He is fluent in speaking English, Urdu and Punjabi.

He is a trainer at both practice, one of the programme directors of the local GP training scheme, the clinical lead at Chapel Medical Centre and a co-clinical director for Windsor Primary Care Network.

Dr Raissa Hamelmann

I qualified as a doctor in 2016 and went on to complete speciality training as a General Practitioner. I have always had a special interest in psychiatry, since completing my training I have been working as a Speciality Doctor in Psychiatry which has allowed me to develop my interest further while striving to provide improved physical and patient-centred health care in a psychiatric inpatient setting.

I believe in the provision of accessible patient care and this is in part what has drawn me to General Practice, as it provides me with a platform to make changes at both a personal and an organisational level – changes inspired with knowledge that I have gained from my MSc in Public Health from the London School of Hygiene and Tropical Medicine. I can speak German and English fluently.

One of my other passions is nutrition and how this affects different interlinked systems in our body to affect the development of conditions such as diabetes and heart disease. Lifestyle medicine and Integrative medicine as a whole medicine form a large part of my practice – I feel that this has enabled me to provide a more holistic approach to support and guide my patients to empower them to make informed healthier choices to manage their long-term conditions.
